Amends Arizona law to allow dentistry to be practiced as a business organization that is registered as a business entity, in addition to professional corporations and professional limited liability companies.

Requires business entities offering dental services to register with the dental board, including disclosure of responsible dentists, officers, directors, and payment of registration fees; registrations expire every three years.

Mandates business entities establish written protocols for secure storage, transfer, and access of patient dental records, with procedures for patient notification, record disposal, and timely response to record requests.

Prohibits individuals with surrendered or revoked dental or dental hygiene licenses from holding majority ownership (greater than 50%) in registered business entities, with a one-year grace period to divest ownership.

Requires conspicuous display of current business entity registration receipts at all practice locations and establishes board disciplinary authority over business entities, including civil penalties up to $2,000 per violation.
