Amends Arizona law to require sample ballots for primary and general elections to be mailed in envelopes with the voter's name printed on the envelope.

Maintains existing requirement that sample ballots be mailed at least 11 days before elections to registered voters not on the permanent early voting list.

Applies the envelope requirement to both county primary elections and general elections, and extends it to city, town, and special district elections.

Requires the state seal and words "official voting materials" to be imprinted on the mailing face of the envelope for each sample ballot.

Clarifies language regarding the printing and distribution responsibilities for sample ballots by local jurisdictions and maintains reimbursement procedures for mailing costs.
