Urges the United States Congress to repeal the requirement that physicians with a National Provider Identifier must enroll in or opt out of Medicare as a condition for receiving payment under federal health care programs.

Identifies inaccuracies in Medicare enrollment systems, including 58% of records in the Provider Enrollment, Chain and Ownership System containing errors and 48% of records in the National Plan and Provider Enumeration System having inaccurate information.

Notes that physicians are increasingly declining to accept Medicare and Medicaid due to government regulations and reduced reimbursement rates, which limits patient facility options.

Directs Arizona's Secretary of State to transmit copies of the memorial to the President of the United States, Congress leadership, and all Arizona members of Congress.
