Amends sections 15-102, 15-701, and 15-741 of Arizona Revised Statutes to establish parental opt-out rights for statewide assessments.

Adds to parental rights notification requirements that parents have the right to opt out of statewide assessments prescribed in chapter 7, article 3 of the education code.

Allows parents to opt out of third-grade reading assessments by providing an alternative assessment method to determine reading proficiency for promotion purposes.

Prohibits schools and districts from factoring missing assessment results from opted-out students into school achievement profiles, report cards, teacher performance systems, or graduation requirements based on statewide assessments.

Requires the state board of education to develop and make available an opt-out form for parents to submit to school principals or head teachers.
