Establishes the Alzheimer's Research Fund under the Department of Health Services to receive voluntary contributions and provide grants to nonprofit and for-profit organizations for Alzheimer's and dementia research.

Requires the Department of Revenue to provide space on individual income tax return forms allowing taxpayers to designate a portion of their refund as a voluntary contribution to the Alzheimer's Research Fund.

Directs the Department of Revenue to subtract designated amounts from taxpayers' refunds and transfer them to the Department of Health Services for deposit in the fund, with initial administrative costs transferred to the state general fund.

Allows taxpayers to make additional donations to the fund by indicating the amount on their tax return and including payment with the return, separate from or in addition to refund designations.

Applies retroactively to taxable years beginning after December 31, 2014.
