Establishes first responder special license plates in Arizona, contingent on a person paying $32,000 to the department for implementation by December 31, 2015, with the designer subject to departmental approval.

Sets the special plate fee at $25 annually, consisting of $8 for special plate administration (deposited to state highway fund) and $17 as an annual donation (deposited to new first responder special plate fund).

Creates the first responder special plate fund to reimburse the initial $32,000 implementation cost, allocate remaining monies (after no more than 10% for administrative costs) to qualified charitable organizations providing assistance to families of injured/killed public safety officers and firefighters and education scholarships, with no more than 10% annually for fund administration.

Requires the designated charitable entity to have existed for at least 45 years and to offer immediate financial assistance, emotional support, peer training, professional referral services, and educational scholarships to families of public safety officers and firefighters.

Allows first responder special plates to be combined with personalized plates at combined fees and amends related statutes to include section 28-2452 in provisions governing special plate administration fees and donations.
