Department of Corrections must submit quarterly reports to the governor and legislature detailing solitary confinement use, including number of prisoners placed, removed, and currently held in solitary confinement at each facility.

Reports must include demographic information (race, gender, age groups under 21 and under 18, and prisoners 56 and older) and data on self-harm incidents, use of force incidents, and out-of-cell programming participation.

Reports must track individual prisoner details including mental health status, reason for confinement, length of stay, hearing procedures, and amount of out-of-cell time, programs, and services provided.

Department must assess all policies and practices regarding solitary confinement use for prisoners with serious mental illness or significant mental impairment, and report on steps taken to reduce solitary confinement use, duration, and social isolation.
