Amends Arizona Revised Statutes section 16-912 to require political committees making independent expenditures to disclose the names and telephone numbers of corporations, limited liability companies, and labor organizations making the three largest contributions to the committee, in addition to existing disclosure requirements.

Expands disclosure requirements beyond political committees to include corporations, limited liability companies, and labor organizations as entities whose contributions must be identified in campaign literature and advertisements.

Maintains existing requirements that campaign literature and advertisements include "paid for by" statements with the entity name and a statement that the communication is not authorized by any candidate or campaign committee.

Applies the expanded disclosure requirements only to contributions made during the calendar year in which the independent expenditure is made.

Retains existing civil penalties of up to three times the cost of producing and distributing literature or advertisements for violations of disclosure requirements.
