Amends Arizona Revised Statutes Section 36-889 regarding child care facility licensees and applicants.

Changes language in subsection B from "other state" to "another state" regarding denial or revocation of child care licenses.

Requires applicants and licensees to maintain a list of controlling persons on file with the department and prohibits issuance or renewal of licenses if any controlling person has been denied a certificate or license for child care facilities or had one revoked for endangering children's health and safety.

Requires applicants and licensees to notify the department within 30 days of any changes to controlling persons and to provide names, addresses, and affirmations regarding the background of new controlling persons.

Requires each applicant or licensee to designate an authorized agent who must be a controlling person, U.S. citizen or legal resident alien, and resident of Arizona to receive departmental communications and legal documents.
