Amends Arizona Revised Statutes section 28-669 concerning accident report forms prepared and supplied by the department to law enforcement agencies.

Requires written accident reports from investigating officers to call for sufficiently detailed information about traffic accidents including cause, existing conditions, and persons and vehicles involved.

Specifies that accident reports must be made on the department-approved form and contain all required information unless such information is unavailable.

Authorizes the department to modify written accident report forms in conjunction with state and local law enforcement agencies, engineering agencies, and emergency medical service organizations.
