Amends Arizona election law to require sample ballots mailed to registered voters be printed in envelopes with the voter's name printed on the envelope.

Modifies section 16-461 to require sample primary election ballots be mailed in envelopes bearing the voter's name and updates language regarding ballot preparation and distribution.

Modifies section 16-510 to require sample general election ballots be mailed in envelopes bearing the voter's name and updates language regarding ballot preparation and distribution.

Changes mailing requirements from sending sample ballots to "households containing a registered voter" to sending to individual registered voters with their names on the envelope.

Applies personalized envelope requirement to sample ballots for primary elections, general elections, and special district elections.
