Amends section 16-584 of Arizona Revised Statutes regarding procedures for provisional ballots cast by electors whose names are not on the precinct register.

Allows qualified electors to vote provisional ballots upon presentation of identification verifying their identity and residence address within the precinct, or by signing an affirmation of voter eligibility.

Requires county recorder to verify provisional ballots within 10 calendar days after federal elections and within 5 business days after other elections, and to use provisional ballot information to correct voter address records.

Establishes procedure for voters appearing at incorrect polling places, allowing them to receive a form directing them to the correct precinct and cast a provisional ballot that counts only for non-precinct-specific candidates and issues if verification confirms the error.

Requires county recorder to provide voters with notification at no cost regarding whether their provisional ballot was verified and counted, through mail, toll-free telephone number, internet access, or similar method.
