Mandates that all independent expenditure advertisements and literature include a clear disclosure statement stating "paid for by" the entity making the expenditure and that it is not authorized by any candidate or campaign committee.

Exempts entities recognized under Internal Revenue Code section 501(c) from the requirement to file as a political committee even if they make independent expenditures and accept donations.

Imposes civil penalties of up to three times the total expenditure amount for failure to register, notify, or disclose as required, and classifies knowingly false filings as a class 1 misdemeanor.

Establishes procedures for notarized sworn statements of authority, categorizes unverified or delinquent filings, and allows local election jurisdictions to make reasonable cause determinations for violations.
