Amends Arizona education law to prohibit school districts and charter schools from distributing personally identifiable pupil information beyond the school district or charter school, including noncognitive data such as birth weight, religious affiliation, political affiliation, and family organization participation.

Allows distribution of aggregated cognitive data (test scores, grades, academic performance measurements) to the Arizona Department of Education, state board of education, superintendent of public instruction, U.S. Department of Education, and other governmental entities.

Requires school districts to adopt procedures for administering surveys to pupils, including advance parental notification, written parental permission before survey administration, full survey text disclosure, identification of all recipients of survey results, and description of survey purpose.

Establishes disciplinary procedures including potential termination of employment for teachers and administrators who violate survey administration and parental notification requirements.

Grants parents the right to commence civil action for damages in superior court against persons or entities violating survey-related requirements, with potential recovery of litigation costs, attorney fees, and expert witness fees.
