Exempts motorcycle, all-terrain vehicle, and motor driven cycle operators and passengers age 18 and older from mandatory helmet requirements if the owner pays a fee upon vehicle registration.

Directs the Department of Economic Security to determine the fee amount, with revenue deposited into the Arizona Highway User Revenue Fund.

Imposes a $500 civil penalty for helmet violations, with $200 deposited to the Arizona Highway User Revenue Fund and $300 to the Spinal and Head Injuries Trust Fund.

Prohibits peace officers from stopping or citing drivers age 18 and older for helmet violations unless there is reasonable cause to believe another motor vehicle law violation has occurred.

Requires that monies from helmet violation penalties deposited to the Spinal and Head Injuries Trust Fund be used only for head injuries resulting from vehicle-related accidents and for rehabilitation services and equipment.
