Mandates disclosure statements on all literature and advertisements funded by these entities, including the words "paid for by" followed by the entity name and a statement that it is not authorized by any candidate.

Requires disclosure statements to identify the four largest major funding sources of the entity at the time of advertisement production, and to specifically identify out-of-state contributors or contributor groups.

Establishes penalties for violations including civil liability up to three times the total expenditure amount and criminal penalties of class 1 misdemeanor for knowingly false filings.

Specifies disclosure format requirements for different media types: radio (spoken at end), printed materials (printed clearly and conspicuously), and broadcasts (both written and spoken, with written text at least 4% of vertical picture height).
