Requires corporations, limited liability companies, and labor organizations to register and notify filing officers within one day of making contributions exceeding specified thresholds: $5,000 in statewide races, $2,500 in legislative races, or $1,000 in local races.

Registration must include the entity's name and address, plus the name, title, email, and phone number of the person authorizing the contribution.

Each contribution notification must disclose the amount, recipient, candidate name, race, support/opposition status, and contribution date.

Entities must file a notarized sworn statement within five days confirming the person filing had authority to make the contribution, or the notification is marked unverified or delinquent.

Violations result in civil penalties of up to three times the contribution amount, and knowingly false filings constitute a class 1 misdemeanor.
