Adds a counseling exception to Arizona's parental consent requirement for minors seeking abortion, allowing minors to bypass parental consent if a physician or counselor provides specific pregnancy information and counseling.

Requires the counselor or physician to objectively explain pregnancy options (carrying to term, adoption, prenatal/postnatal care, and abortion), discuss involving parents or guardians, and confirm the minor understands she may withdraw her abortion decision at any time.

Mandates the minor sign a form acknowledging receipt of counseling information covering alternatives to abortion, explanation of decision reversal rights, exploration of pregnancy management options, and discussion of parental involvement.

The counselor or physician must sign the form with their address and telephone number, maintain a copy, and provide or transmit the form to the attending physician.

Preserves existing parental consent requirement for minors unless they qualify under the medical emergency exception or the new counseling exception under this provision.
