Establishes licensure requirements for body art establishments (tattoo, piercing, branding, scarification) beginning January 1, 2018, administered by local public health departments.

Prohibits body art on minors under 18 without written consent and physical presence of parent or legal guardian.

Requires single-use sterilized needles for tattooing and body piercing; restricts piercing guns to ear piercing only and requires disinfection between uses.

Mandates minimum state standards for sanitation, equipment sterilization, pest control, bloodborne pathogen training (twice yearly), and recordkeeping as conditions for licensure.

Allows local health departments to investigate violations, issue cease and desist orders, impose civil penalties, and suspend licenses; permits cities, towns, and counties to adopt more stringent regulations.
