Allows physicians whose licenses expired to reapply within two years by submitting a renewal form, paying renewal and late renewal fees, and providing proof of required continuing medical education.

Requires physicians reapplying after license expiration to submit a notarized statement describing any medical practice conducted after their license expired.

Authorizes the Arizona Medical Board to renew expired licenses upon satisfactory completion of all requirements.

Empowers the board to assess civil penalties up to $1,000 or impose other sanctions, including denial of the application, if the physician engaged in unauthorized medical practice after license expiration.
