Requires written parental consent before schools collect personally identifiable information on students and written consent from teachers before collecting their data.

Prohibits sharing of student or teacher personally identifiable information with federal agencies, federal personnel, vendors, contractors, or nongovernmental entities without prior written parental or teacher consent.

Bans collection of specified student data including juvenile/criminal records, biometric information, political affiliation, religion, firearm ownership, drug/alcohol use, mental health screening data, and interpersonal relationship information.

Prohibits adoption of assessments that collect psychological data on noncognitive skills, attributes, emotions, values, attitudes, beliefs, motivations, or other affective domain information.

Establishes civil penalties up to $5,000 per violation for state employees and up to $10,000 per student record for other entities; prohibits use of public funds to pay penalties and bars violators from state contracts.
