Adds section 36-2930.02 to Arizona Revised Statutes to establish grounds for excluding providers from participation in the Arizona Health Care Cost Containment System (AHCCCS).

Mandates exclusion of any individual or entity meeting bases for mandatory exclusion under 42 Code of Federal Regulations section 1001.101.

Permits the administration discretionary authority to exclude providers for permissive exclusions under federal regulations, violations of sections 36-2918 or 36-2957, patient neglect resulting in death or injury, unlawful medical waste disposal, abortion-related billing violations, failure to segregate taxpayer dollars from abortion expenses, or failure to comply with mandatory reporting laws for abuse and trafficking.

Requires the director to adopt rules establishing procedures for determining exclusion length, appeal processes, and reinstatement requests.

Defines "exclude" as nonreimbursement of items and services by AHCCCS or its contractors, including termination or refusal to enter into provider agreements.
