Requires the Arizona Department of Health Services administration to establish and implement a uniform patient reporting system for all behavioral health services, including mental health treatment, substance abuse treatment, and related hospital services.

Mandates service providers and subcontractors report statistical data on service utilization, effectiveness, and cost containment, including number of encounters, charges per service, and length of stays categorized by diagnosis.

Requires reporting of outpatient service data including dates of service, service lengths and numbers, charges, and clinical demographics (age, gender, ethnicity, insurance coverage) for inpatient and emergency department services.

Establishes reporting deadlines of August 15 for data covering January-June and February 15 following year for data covering July-December.

Makes all submitted reports publicly accessible while ensuring confidentiality protections for privileged medical information and protected health information; authorizes the administration to investigate accuracy of reports and charge investigation costs to facilities found significantly deficient or incorrect.
