Urges the United States Congress to repeal the health insurance tax imposed by sections 9010 and 10905 of the Patient Protection and Affordable Care Act and section 1406 of the Health Care and Education Reconciliation Act of 2010.

Asserts the health insurance tax will increase individual market premiums by an average of $2,150 nationally over ten years and increase Arizona premiums by an average of $1,964 over ten years.

Claims the tax will reduce private sector jobs by 125,000 over ten years, with 59% of reductions affecting small businesses, and reduce potential sales by at least $18 billion.

States the tax will increase premiums for Medicare Advantage beneficiaries in Arizona by an average of $3,303 over ten years and cost Medicaid beneficiaries in coordinated care programs an average of $1,337 over ten years.

Directs the Arizona Secretary of State to transmit copies of the memorial to the President, Speaker of the House, President of the Senate, and all Members of Congress from Arizona.
