Amends Arizona Revised Statutes Section 16-912 to expand disclosure requirements for independent political expenditures on campaign literature and advertisements.

Requires political committees making independent expenditures to disclose the names and telephone numbers of the three largest contributing entities, now including corporations, limited liability companies, and labor organizations (previously only political committees).

Maintains existing requirements that campaign literature and advertisements include "paid for by" statements with the committee name and disclosure that the communication is not authorized by any candidate.

Applies disclosure requirements to all forms of campaign communications including radio broadcasts, printed materials, and telecommunications broadcasts, with specific formatting and display standards.

Subjects violations to civil penalties of up to three times the cost of producing and distributing the non-compliant literature or advertisement.
