Amends four Arizona insurance statutes (20-841.09, 20-1057.13, 20-1376.05, and 20-1406.05) to require coverage of telemedicine health care services for HMOs, health care services organizations, disability insurers, and group/blanket disability insurers.

Extends telemedicine coverage requirements effective date from January 1, 2015 to January 1, 2018 for all newly issued, delivered, or renewed insurance policies and evidence of coverage.

Removes the "rural region" limitation, eliminating the previous geographic restrictions that limited telemedicine coverage to rural areas with populations under 900,000 or cities/towns more than 30 miles from larger population centers.

Adds pulmonology to the list of covered health care services provided through telemedicine, which previously included trauma, burn, cardiology, infectious diseases, mental health disorders, neurologic diseases, and dermatology.

Maintains that telemedicine coverage applies only if the underlying health care service would be covered in-person, and allows insurers to impose equivalent deductibles, copayments, or coinsurance as in-person consultations; effective December 31, 2017.
