Requires prior written consent from parents/guardians before schools or the Arizona Department of Education can collect or share any personally identifiable student or teacher data, with a written explanation of why the information is requested and how it will be used.

Prohibits collection of psychological or affective domain data (emotions, values, beliefs, attitudes, social/interpersonal skills) through any state or national student assessment.

Bans sharing of student or teacher personally identifiable information with federal agencies, nongovernmental entities, vendors, or contractors without prior written parental consent; prohibits sharing with the U.S. Department of Education.

Specifically prohibits collection of juvenile delinquency records, criminal records, biometric information, political affiliation, religion, firearm ownership, drug/alcohol use, mental health data, and quality of interpersonal relationships of students, siblings, or parents/guardians.

Establishes civil penalties up to $5,000 per violation for state employees and $10,000 per student record for contractors or other entities that violate the requirements; allows attorney general enforcement and contract termination.
