Mandates contractors provide parents or guardians with an information sheet after each visit containing the dentist's name, description of treatment rendered with service codes and fees, referral information if needed, and provider contact information.

Requires contractors to provide a copy of the patient information sheet to any institutional facility that has been authorized by the parent or guardian to access the patient's dental health records.
