Requires disability insurers in Arizona to cover lawful health care services provided by health care providers regardless of familial relationship to the insured, effective July 1, 2017.

Coverage must be provided on the same basis as if the health care service were provided by an unrelated provider.

Insurers may limit coverage to health care providers within their provider network.

Applies to individual disability policies and group or blanket disability policies issued, delivered, or renewed on or after July 1, 2017.

Excludes disability income coverage from these requirements.
