Requires stroke registry data to align with consensus metrics from the Centers for Disease Control and Prevention's Paul Coverdell National Acute Stroke Registry, Joint Commission, American Heart Association, and American Stroke Association.

Mandates comprehensive stroke centers and primary stroke centers to report stroke treatment data; encourages acute stroke ready hospitals and emergency medical services agencies to participate in reporting.

Requires the department to compile an annual report on the stroke registry's operation, use, and collected data to improve quality of care and facilitate information sharing among medical facilities and healthcare providers.

Uses a data management platform established by the American Heart Association and American Stroke Association or an equivalent platform with substantially equivalent security standards.
