Removes requirement that school immunization records be part of the mandatory permanent student record.

School districts and charter schools must maintain immunization records for six years after a pupil's last enrollment, or six years after high school graduation or until age 21, whichever is later.

Schools must provide copies of immunization records at no charge to parents or guardians upon request.

By November 30 each school year, school districts, charter schools, and private schools must report to local health departments the number of students with completed immunizations, incomplete immunizations, and immunization exemptions.

Standardized immunization record forms are developed by the Department of Health Services in conjunction with the Department of Education and remain open for inspection by health departments.
