Amends Arizona law to require death certificates to record a decedent's sex based on their gender identity, as reported by the person completing the certificate.

Allows the recorded gender identity to be based on documentation memorializing the decedent's gender transition, including court orders, written instructions, healthcare directives, clinical treatment records, or changes to birth certificates, driver licenses, or passports.

If no transition documentation is presented and those with the right to control the remains disagree with the reported gender identity, the certificate shall reflect the gender identity as reported by the person or majority of persons controlling the disposition of remains.

Permits local registrars and state registrars to amend a registered death certificate's sex marker upon presentation of documentation memorializing a gender transition.

Provides immunity from civil liability and professional disciplinary action for healthcare providers who complete medical certifications of death in good faith.
