Amends Arizona Revised Statutes section 28-669 regarding accident report forms.

Clarifies that written reports required to be made by investigating officers must call for detailed information about traffic accidents including cause, conditions, and persons/vehicles involved.

Requires accident reports to be made on department-approved forms containing all required information unless such information is unavailable.

Authorizes the department to modify written accident report forms in coordination with state and local law enforcement agencies, engineering agencies, and emergency medical service organizations.
