Allows voters who move within the same county and fail to update their address before election day to correct their registration at the polling place for their new address and vote a provisional ballot if the new address is verified to be in that precinct.

Requires election boards to direct voters whose residence address is not in the precinct where they are attempting to vote to their correct precinct, and if they refuse, to issue a provisional ballot with a warning that only eligible votes will be counted.

Mandates the Secretary of State design a provisional ballot envelope template that includes a section to record the correct precinct the voter was directed to, with signature lines for both the voter and election board official to confirm the precinct information and warnings were communicated.

Establishes that provisional ballots cast in the correct county but wrong precinct shall be opened and only votes the voter was eligible to cast in their actual precinct shall be counted, while ballots cast outside the voter's county shall be rejected entirely.

Requires counties to provide voters with notification at no cost of whether their provisional ballot was counted, and if not counted, the reason, through mail, toll-free phone, internet access, or similar methods.
