Requires health insurance plans, health service organizations, disability insurance policies, and blanket disability policies that cover prescription drugs to include coverage for all FDA-approved contraceptive drugs and devices, including oral, implant, and injectable contraceptives, intrauterine devices, and prescription barrier methods.

Prohibits insurance plans from imposing deductibles, coinsurance, copayments, or other cost-sharing requirements for contraceptive drugs, intrauterine devices, prescription barrier methods, or male sterilization procedures.

Requires coverage for outpatient contraceptive services including consultations, examinations, procedures, and medical services related to the use of FDA-approved prescription contraceptive methods.

Allows religiously affiliated employers to opt out of contraceptive coverage requirements by filing a written affidavit with the insurer, provided the employer meets specific criteria regarding religious beliefs and organizational structure.

Excludes nongroup (individual) insurance policies from these contraceptive coverage requirements.
