Requires the Arizona Department of Health Services director to apply annually by March 30 for federal waivers allowing work requirements for able-bodied adults receiving AHCCCS (Arizona health insurance) benefits.

Work requirement must include either employment, active job seeking, or attending school or job training for at least 20 hours per week, verified monthly by the department.

Imposes a five-year lifetime benefit limit for able-bodied adults, excluding time spent pregnant, as sole caregiver of children under six, receiving disability benefits, or attending high school.

Establishes cost-sharing requirements to discourage nonemergency emergency department use and unnecessary ambulance services.

Exempts American Indians and Alaska natives eligible through Indian Health Service or tribal health programs from all waiver requirements.
