Establishes a diabetes action plan team within the Department of Health Services composed of representatives from the Arizona Diabetes Program, Arizona Health Care Cost Containment System, Public Safety Personnel Retirement System, Arizona State Retirement System, Department of Administration Benefits Services Division, and diabetes stakeholder organizations.

Requires the team to compile a biennial report (due January 1, 2019 and every two years thereafter) that includes diabetes prevalence by type, age, race and gender; diabetes complications; prediabetes prevalence; and costs of diabetes in Arizona.

Report must include the Arizona Diabetes Program's plan for reducing diabetes incidence, improving care, and reducing health disparities, with specific action steps.

Report must describe coordination levels between the Department of Health Services, hospitals, contracted partners, and stakeholders on diabetes management, treatment, and prevention activities.

Data collection requirements are limited to existing diabetes information, data, initiatives and programs within each agency unless unobligated diabetes funding is available for new research and data collection.
