Requires referral agencies to disclose to prospective residents or their representatives the existence of any business relationship with the assisted living facility, including common ownership, financial relationships, and familial connections.

Mandates disclosure of the referral fee amount or a good faith estimate before or at the time of the resident's admission date.

Requires both the referral agency and prospective resident (or representative) to sign and date or electronically acknowledge the disclosures, with copies provided to all parties and maintained on file at the facility.

Prohibits assisted living facilities from paying any referral fee until they receive the required disclosures from the referral agency.

Subjects referral agencies to civil penalties of up to $1,000 per violation, enforceable by the Attorney General or county attorney, with penalties deposited in the general fund.
