• Requires health insurance contracts providing prescription drug coverage to include coverage for all FDA-approved contraceptive drugs and devices, including oral, implant, and injectable methods, intrauterine devices, and prescription barrier methods without greater cost-sharing than other drugs at the same formulary tier.

• Mandates coverage for up to a 12-month supply of self-administered contraceptives dispensed at patient request with a valid prescription allowing initial quantity and periodic refills.

• Requires coverage for outpatient contraceptive services including consultations, examinations, procedures, and medical services related to FDA-approved prescription contraceptive methods.

• Allows religiously affiliated employers to exclude contraceptive coverage by filing a written affidavit with the insurer, but exempts contraceptives prescribed for non-contraceptive medical purposes and protects employees' health information privacy.

• Applies to hospital service corporations, health care services organizations, group disability insurers, blanket disability insurers, and disability insurance policies with prescription drug coverage.
