Requires death certificates to record the decedent's sex to reflect their gender identity unless a document memorializing gender transition is provided, in which case that document controls.

If no gender transition document is presented and the person(s) with right to control the disposition of remains disagrees with the reported gender identity, the gender identity recorded on the death certificate shall be as reported by those person(s).

Allows local registrars, deputy local registrars, or the state registrar to amend the sex on a registered death certificate when presented with documents memorializing gender transition, including court orders, written instructions, health care directives, clinical treatment records, or changes to birth certificates, driver licenses, social security records, or passports.

Reorganizes existing provisions regarding death certificate registration timelines, medical certification requirements, and procedures for determining cause of death and place of death.

Provides immunity from civil liability and professional disciplinary action for health care providers who complete and sign medical certifications of death in good faith.
