Requires school district governing boards to develop and provide guidelines, information, and forms in consultation with statewide interscholastic activity supervisors to educate coaches, pupils, and parents about dangers of concussions, head injuries, heat-related illnesses, sudden cardiac death, and prescription opioid use.

Mandates that pupils and parents sign an information form at least once per school year acknowledging awareness of concussion risks before participation in any district-sponsored athletic practice, game, or interscholastic activity.

Requires immediate removal from play of any pupil suspected of sustaining a concussion; allows return to play same day only if a health care provider rules out concussion, or on subsequent days with written clearance from a trained health care provider.

Provides immunity from civil liability for school districts, employees, volunteers, coaches, officials, and volunteer health care providers acting in good faith to comply with concussion management requirements, except in cases of gross negligence or willful neglect.

Requires pupils and parents to receive information at least once per school year on risks of heat-related illnesses, sudden cardiac death, and prescription opioid addiction before participating in any district-sponsored athletic activities.
