Amends A.R.S. 36-2411 to establish requirements for custom orthotic and prosthetic devices provided to individuals beginning January 1, 2018.

Requires fabricators and suppliers to be accredited by an independent organization approved by the U.S. Department of Health and Human Services, unless the provider is licensed under specified Arizona healthcare professional chapters.

Mandates that custom orthotic or prosthetic devices be subject to a valid prescription and provided by qualified practitioners or licensed healthcare providers.

Prohibits health care providers from attempting to collect payment from patients for denied claims when the denial is strictly based on the device or provider not meeting accreditation requirements.

Clarifies that the statute does not prevent licensed healthcare providers from issuing prescriptions or practicing their profession, provided they do not bill insurance or collect patient reimbursement for such devices.
