Creates a statewide uniform video service licensing system in Arizona for cable and video service providers, preempting local regulation effective December 31, 2019.

Establishes that local governments must adopt standard uniform video service license agreements by July 1, 2019, with licenses issued within 30 days of completed applications without local government approval votes required.

Limits local government fees to a 5% combined rate on gross revenue for license fees and transaction privilege taxes, with additional highway-related fees permitted if offset against license payments.

Allows incumbent cable operators to either continue operating under existing local licenses or terminate and convert to uniform video service licenses by July 1, 2020.

Establishes the Office of Administrative Hearings to hear complaints from video service providers against local governments and vice versa, with appeals to superior court available.
