Requires the Department of Education to submit an annual report by September 1 (starting in 2019) to state leadership containing statewide disciplinary data disaggregated by school district, charter school, and demographic subgroups.

Mandates the report include disciplinary rates for in-school suspensions, out-of-school suspensions, expulsions, and law enforcement referrals, calculated as a three-year average for each measure.

Requires the Department of Education to track progress in reducing disciplinary rates and disparities, identify successful strategies used by schools, and assess corresponding gains in student academic achievement.

Directs the Department to survey schools about implementation of evidence-based discipline strategies including positive behavior interventions and support systems and restorative justice practices.

Defines "subgroup" to include demographic categories by race/ethnicity, low-income students, students with disabilities, and students with below-basic academic achievement levels for comparative analysis of disciplinary disparities.
