Establishes licensure and regulation of dental therapists in Arizona, requiring applicants to be licensed dental hygienists who complete an accredited dental therapy education program and pass clinical and jurisprudence examinations.

Authorizes dental therapists to perform specific clinical procedures including oral evaluations, cleanings, restorations, extractions of certain teeth, and administration of local anesthetics and nitrous oxide under a written collaborative practice agreement with a supervising dentist.

Requires dental therapists to practice under written collaborative practice agreements with supervising dentists; limits dentists in private practice to no more than five separate collaborative practice agreements.

Grants dental therapists authority to practice dental therapy procedures without the dentist being on-site if within ten miles and the supervising dentist has not previously examined the patient, subject to agreement terms.

Amends existing dental board statutes to add dental therapists to definitions, unprofessional conduct provisions, disciplinary authority, and fingerprinting requirements; establishes dental therapist triennial licensure with renewal fees and continuing education requirements.
