Amends section 8-513 to require the out-of-home provider for youth at least 16 years old to work with independent living programs focused on career, education, and future development planning to assist youth in meeting program goals.

Establishes requirements for child welfare agencies to obtain and provide children at least 16 years old with access to documents (birth certificate, nonoperating identification license, immunization records) within 60 days of placement.

Allows licensed foster homes and kinship foster homes to voluntarily obtain and provide access to required documents for youth at least 16 years old.

Requires the Department of Child Safety to provide foster parents and kinship foster parents with a child's social security number within 90 days of request for lawful purposes, and to provide children at least 14 years old with their social security card within 120 days of written request.

Requires the Department of Child Safety to request a birth certificate for children placed in out-of-home care within 30 days of placement unless the child is returned to parental custody within that timeframe.
