Amends Arizona Revised Statutes section 21-314 to authorize the jury commissioner or jury manager to investigate the accuracy of answers provided on juror questionnaires.

Allows law enforcement agencies and the county attorney to assist in investigations of questionnaire accuracy.

Permits the jury commissioner or jury manager to destroy fully answered questionnaires 90 days after receipt.

Allows questionnaires to be mailed either with the summons to appear for jury duty or separately in advance.
