Establishes state licensure requirements for sober living homes through the Department of Health Services, with standards to be adopted by rule within two years of enactment.

Allows sober living homes certified by recognized national certifying organizations to operate without initial licensure until they apply for state licenses within 90 days after rules are finalized.

Requires sober living homes to develop policies allowing residents on medication-assisted treatment to continue that treatment, maintain supervision during all operating hours, establish operation plans addressing rehabilitation and discharge planning, and comply with federal fair housing laws and the Americans with Disabilities Act.

Prohibits cities, towns, and counties from restricting vacation rentals or short-term rentals from being used as sober living homes, and removes the word "structured" from existing sober living home definitions in city and county ordinances.

Requires state agencies, courts, and licensed healthcare providers to refer individuals only to certified or licensed sober living homes beginning January 1, 2019, and prohibits disclosure of sober living home addresses except to law enforcement, emergency personnel, and local jurisdictions for zoning purposes.
