Requires Arizona's health care director to annually apply for federal waivers to implement work requirements for able-bodied adults receiving AHCCCS benefits, including employment, job seeking, or attending school/training at least 20 hours per week.

Establishes a five-year lifetime benefit limit for able-bodied adults, with exemptions for pregnant individuals, primary caregivers of children under six, those receiving disability benefits, high school students age 19+, and those enrolled before age 19.

Mandates monthly verification of work requirement compliance and family income changes, with authority to ban individuals for one year if they knowingly fail to report income changes or make false statements.

Exempts American Indians and Alaska natives eligible through Indian Health Service or tribal/urban Indian health programs from work requirements and lifetime limits.

Authorizes cost-sharing requirements to discourage nonemergency use of emergency departments and unnecessary ambulance services.
