Establishes an underground injection control (UIC) permit program administered by the Department of Environmental Quality, with permits required for underground injection wells except those authorized by rule or federal permit.

Prohibits underground injection that allows contaminant movement into underground sources of drinking water that may endanger drinking water supplies, with exemptions for Class V wells holding aquifer protection permits that satisfy federal UIC requirements.

Applies federal UIC definitions and regulations from the Safe Drinking Water Act (42 U.S.C. § 300h) in effect as of January 1, 2018 to the state's program.

Establishes civil penalties of up to $5,000 per day per violation for UIC permit program violations and adds criminal violations for commencing underground injection without proper permit or authority.

Allows citizens to intervene in state UIC enforcement actions to obtain temporary restraining orders, injunctions, and civil penalties if adversely affected by violations.
